Family Badges
Use this form to submit names and photos of persons who will be receiving the badges for your family. You can submit for up to 3 badges. Photos provided should be similar to that required by any official ID card. Follow the photo guidelines below before uploading an image.
If a photo submission does not follow these guidelines, the individual will be required to resubmit their photo causing a delay in processing.

Photo Guidelines
- Photos must be up to date photo and a true likeness of the individual with their face fully visible. No face coverings, hats, or sunglasses.
- Head must be framed in full, centered, and facing forward.
- Face should be well-lit.
- Photos must be of adequate quality allowing for any personnel to fully identify the individual.
- Check that the photo quality is not distorted or grainy.
- Background should be a solid surface, preferably white. No scenery.
- A natural expression is preferred.
- Submit photos following the composition examples below.
- Avoid submitting casual photos similar to those used in social media.
Photo Composition Examples
Badge entries submitted Monday-Friday before 12:00pm will be available for pickup the same day at the Front Office.
Badge entries submitted on weekends or after 12:00pm will be available for pickup the following business day at the Front Office.
