Use this form to submit your name and photo to receive a Faculty or staff badge. Follow the photo guidelines before uploading an image.

Photo Guidelines

• Photo must be a true likeness of the recipient with their face fully visible.
• Head must be framed in full, centered, and facing forward.
• Lighting should be even.
• Background should be a solid surface, preferably white, rather than scenery.
• A natural expression is preferred. 
• Only jpg and png file types are accepted.
